This medal was established by Maharaja Bhupendra Singh. It was conferred upon Patiala State Forces and Patiala Police Force personnel in recognition of notable and meritorious service.

The reverse features an inscription in Gurmukhi and Devanagari, which translates to "Meritorious Service".
