The Medal was founded by King Wilhelm II to commemorate the 100th birthday of his grandfather, Emperor Wilhelm I. It was also known as the "Centenary Medal" and "Commemorative Medal for the Most Blessed Emperor and King Wilhelm I, the Great".

The medal was conferred upon all active members of the military, state and university officials, veterans of the Unification Wars (First Schleswig War, Second Schleswig War, Austro-Prussian War, and the Franco-Prussian War), and female members of the Order of Louise.
