The Life Saving Medal was instituted in 1868 by Prince Günther Friedrich Carl I. It was conferred upon individuals who risked their own lives to save others from life-threatening danger.

In 1880, Prince Karl Günther became the ruler of Schwarzburg-Sondershausen and the medals were altered to feature his image on the obverse.
