The Medal for Horse Care was instituted by the Commander of the Freikorps von Diebitsch, Oberstleutnant Karl Heinrich von Diebitsch, in 1919. It was conferred upon Freikorps military personnel who took exceptional care of the unit horses.

The medal is round and silvered. The obverse features a horse head facing to the viewer's left, with a branch of laurel below it. This is identical to the one on the Landesjägerkorps Medal for Horse Care. The semi-circular inscription along the top half translates to "For Good Horse Care".
The reverse features the Star of the Black Eagle Order with a ribbon below it that reads "FREIKORPS V. DIEBITSCH".

The ribbon is golden-yellow with three black stripes.

The medal was awarded approximately 100 to 150 times.
