The Transport Medal was sanctioned on November 8, 1903. The Silver Medal was awarded by the Lord Commissioner of the Admiralty to Officers, masters, engineers, surgeons, and pursers of merchant vessels used to carry troops and supplies to the Boer War in South Africa between 1899 and 1902 and the Boxer Rebellion in China in 1900.

A “S. AFRICA 1899-1902” and a “CHINA 1900” clasp were authorized for wear with the Silver Medal.

The Medal features a Latin inscription on the reverse which translates to, "For Services Rendered in Transporting Troops by Sea."
