The award is in the form of a rectangular silvered plaque, featuring a Luftwaffe eagle with swastika facing to the viewer's right over an anti-aircraft cannon aiming to the viewer's left, superimposed over a map of the Netherlands. In the centre at the bottom the plaque shows the city crest of Amsterdam, with "Nieder" and "Lande" and "1940" and "1941" to the left and right, respectively.

The German title of this award is "Ehrenplakette des Luftgaukommandos Holland".
