The Medal was instituted by King Ernst August and was conferred upon individuals who put their own life in peril in order to rescue another person.

The first issue of the medal (1845-1846) features a small head of King Ernst August the name "FICKENSCHER" under the neck.

The second issue of the medal features a larger head of King Ernst August and the name "BREHMER F." under the neck.
