This medal was conferred upon Chief Warders, Head Warders and Warders of the South African Prisons Department for distinguished service. It was also awarded for 18 years of impeccable service.

A clasp was awarded for bravery.

This medal was originally known as the "Prisons Good Service Medal" (1922-1959). It was discontinued in 1968 as it was the first of three series of decorations for the South African Prisons Department.

The reverse features an inscription that varies: a) FOR FAITHFUL SERVICE - VOOR TROUWE DIENST (1922-1932); b) FOR FAITHFUL SERVICE - VIR TROUE DIENS (1932-1959); c) VIR TROUE DIENS - FOR FAITHFUL SERVICE (1959-1968).
