The obverse of the medal features a stylized Polar Star, surrounded by wings. It is inscribed with [ORDER OF POLARIS], with a single maple leaf on either side. The reverse feature the Yukon Territories coat of arms and is inscribed with [YUKON].

Aviators who have flown within the Yukon are awarded a bar.

The medal was last instituted in 2011.
