The Surgeon General's Medallion is awarded at the sole discretion of the Surgeon General to an Officer of any of the uniformed services, or to civilians, in recognition of significant contributions made to the initiatives of the office of the Surgeon General and to public health and medicine. Contributions may include medical breakthroughs in public medicine, disease prevention and control, or exceptional service in a senior position of health and human services.

The Medal is similar in design to the Surgeon General Exemplary Service Medal.

There are 2 versions of the award. The award is generally awarded to Officers as a medal suspended from a ribbon. A table medallion version of the award also exists, which features a blank reverse. It is believed that the table Medal is awarded to civilians, however, more research is necessary.
