The Medal for Outstanding Achievement in Sport was established by the Polish People's Republic in 1954 but was discontinued following the collapse of the Polish People's Republic in 1989. It was awarded to Polish citizens in recognition of achievements in sport on the national or international stage.

The I Class Decoration was awarded to athletes who had won gold at the Olympics, world championships, or European championships. It was also awarded in recognition of setting new world or European records.

The I Class Decoration features a reverse inscription that can be translated to "For Outstanding Achievement in Sport."
