The Balkan Alliance Medal was awarded to "commemorate the alliance between Serbia, Montenegro, Bulgaria and Greece".

The obverse of the medal shows s head and shoulders portrait of King Petar I Karađorđević in uniform. It circumscribed ‘ПЕТАР I КРАЉ СРБИЈЕ’ (translates to "PETAR I KING OF SERBIA"). The medal is signed ‘ПИХЛЪ’ (Pichl – for Joseph Bojislav Pichl) and inscribed ‘ПРАГА’ (Prague).

The reverse with a central Patriarchal cross on a crown of thorns imposed on a laurel wreath and radiant flaming rays, circumscribed ‘IN HOC SIGNO VINCES’ (Latin = In this sign you shall be victorious)
