This medal was awarded to personnel who served under Milosh Obrenovich and for long service to the household. At a later date the medal was also awarded to veterans of the Turkish and Bulgarian wars.

The obverse of this medal translates to "GLORY TO THE GREAT MILOSH GLORY TO THE ST. ANDREWERS".
