A medal in the shape of a shield with a wreath on the top, the obverse bears the bust of Alfonso XII with the inscription ALFONSO XII REY DE ESPANA, the reverse bears the coat of arms of various provinces with Madrid in the center, encompassed by the inscription LA PROVINCIA DE MADRID AGRADECIDA A SUS HIJOS, with a suspension loop on a white ribbon.
