A circular medal constructed of silver, with four fleur de lis distributed around the edge to form a cross, the obverse bears the bust of Alfonso XIII as a child, within a border inscribed with ALFONSO XIII A LOS VOLUNTARIOS DE PUERTO RICO, the reverse bears the inscription CONSTANCIA PATRIOTISMO ABNEGACION, on a red and yellow striped ribbon, the ribbon bears a clasp with the inscription 16 ANOS DE SERVICIO
