The medal was founded by Emperor Franz Joseph to commemorate his time as the honourary leader of the 2nd Royal Prussian "Emperor Franz" Guard Grenadier Regiment.

The medal dies were cut by J. Tautenhayn.

The Silver Medal was awarded to higher ranking members of the Regiment, such as Officers and Generals.
