This Cross was awarded to personnel within the Italian Armed Forces who rendered honourable service within humanitarian missions outside of national borders and aligned with international organizations, such as the United Nations. The Cross may be awarded multiple times for service in several missions. Each bestowal is recognized by the addition of a star to the ribbon. The second mission is represented by a single bronze coloured star, the third mission is represented by two bronze coloured stars, and three or more missions are represented by a single silver coloured star.

The ribbon may also feature clasps denoting the name of the region/country in which the recipient rendered humanitarian services. Known clasps include, "ALBANIA 1," "ALBANIA 2," "MACEDONIA," "IRAQ 2003," "IRAQ 2004," and "INDUS."
