The decoration was instituted in 1942 by General Bernhard Waber. It was accompanied by an award document. Since the Air District Kharkov only existed for about half a year (until April 1943), this award was probably not awarded very often and remains rare.

The plaque is rectangular with a raised edge. It shows a Luftwaffe eagle facing to the viewer's right, with three city crests underneath: Poltawa, Kiew, Kursk. Underneath are two crossed swords and then a larger city crest: Charkow, with laurels over oak leaves on both sides. Underneath that is the inscription, which reads: for special merits / the commanding general and chief of Air District Kharkov.
The reverse is hollow. The plaque weighs approximately 290 grams.

The German title of this award is "Plakette des Luftgaues Charkow".
