The award was instituted by General Hellmuth Bieneck. Details about the institution or the award's purpose are not known. Known examples have been awarded as early as 1938.

The plaque is rectangular with a raised border. The upper half shows a first pattern "droop tail" Luftwaffe eagle with swastika. Underneath is the recipient's rank and name. Below that is the inscription, which translates to: for outstanding merit / the commander of air division 4.

The German title for this award is "Ehrenplakette der Flieger-Division 4".
