The Medal was established by the Senate of Bremen and it was awarded at the discretion of the Senate.

Until 1907, the Medal was awarded to recognize achievement in commerce, trade, agriculture, and animal husbandry.

After 1907, the Medal became an all purpose award for the city of Bremen.

The Iron Medal was added as a grade in 1918, due to the shortage of materials in wartime.
