This medal is also known as "Pingat Pekerti Terpileh" and is awarded for distinguished conduct.

The obverse of this bronze medal bears the design of crossed kris and sheath above the word "Perlis" in Jawi surrounded by padi ears. The reverse is a surround of laurel leaves with the name of the medal in Jawi and the name of the recipient.
