The Medal was founded by the Minister of Defence of the Czech Republic to commemorate the 50th anniversary of the Czech Republic joining NATO.

It was awarded to personnel who were instrumental in the integration of the Czech Republic into NATO, and building communications between NATO and the Czech Republic.
