The medal was conferred in recognition of service with the Red Crescent, the equivalent of the Red Cross within the Ottoman Empire.

The recipient's years of service with the Red Crescent are sometimes inscribed on a white enameled bar on the ribbon. The ribbon also features an oak leaf decoration. The suspension bar features Arabic script, translated as "OTTOMAN RED CRESCENT ASSOCIATION".

The obverse inscription is a rough translation.
