The Cross was originally founded by Grand Duke Ludwig III, but it was renewed by Grand Duke Ernst Ludwig on August 12, 1914.

The Type II Cross was conferred upon individuals, regardless of class or gender, who rendered exemplary service in the care and transportation of sick and wounded soldiers during the First World War.

Sanitation Officers, Officers, and other ranks received the Cross on a war ribbon, all others received the Cross on a ribbon similar to the 1870 Type I Cross.

As the War progressed and materials became scarcer, manufacturers began producing crosses composed of coppered war material.
