The Duke Alfred Medal was instituted on January 1, 1896.
The German name of the medal is "Herzog Alfred Medaille".
The silver circular medal shows on the obverse centre the right-looking portrait of Duke Alfred with full beard, bordered by the inscription "ALFRED HERZOG VON SACHSEN COBURG UND GOTHA".
The reverse presents two crowned lions standing on a banner inscribed "TREU UND FEST", holding a ribbon bearing the inscription "HONI SOIT QUI MAL Y PENSE". In the enclosed field is the Duke’s coat of arms and at the top is the message "FÜR VERDIENST".
