The medal was founded by King Albert and Queen Carola on the 25th anniversary of the Albert Association. It was awarded to volunteer nurses for merit in their training during the Franco-Prussian War of 1870/71.
After the war had ended the medal continued to be awarded during peacetime in recognition of altruism and public services.
On February 26, 1915 the Type III medals were amended, and were awarded to both men and women for charitable services in peacetime and voluntary nursing services during wartime.
The medals awarded for merit in wartime were also awarded with a clasp reading "WELTKREIG 1914-16". The clasps conferred upon women were shaped like an oak leaf, and the clasps conferred upon men were rectangular-shaped with rounded corners.
When the Type III medal was awarded to women it was worn on a bow ribbon.
