The NATO Service Medal was instituted in December of 1994 to reward NATO military and civilian personnel for service in any area of operations. There are multiple versions of the Bronze Medal rewarded in recognition of different operations, with the period of qualifying service varying across campaigns. The operations recognized by the Medal are denoted by various combinations of blue, white, and gold or silver Medal ribbons and clasps.

Medals awarded for service in former Yugoslavia are worn with a ribbon with 2 white stripes near the edge while Medals awarded for service in Kosovo are represented by a ribbon with a center white stripe and 2 narrow white stripes on the edge. These versions were initially issued with, respectively, either a "FORMER YUGOSLAVIA" or "KOSOVO" clasp. However, both clasps have since been superseded with the "NON ARTICLE 5" clasp.

The ribbon worn with the Medal for service in the Republic of Macedonia features 4 white stripes, with the edges being bordered by 2 parallel stripes.

The ribbon worn with Medals issued in recognition of service in Article 5 Operations features a gold thread. Ribbons worn with the Eagle Assist Medal are represented by a single centered white stripe with gold threat and Ribbons worn with the Active Endeavour Medal are represented by 2 white stripes with gold thread. The Eagle Assist and Active Endeavour Medals are issued with the "NON ARTICLE 5 CLASP."

The Medal ribbon awarded for certain Non-Article 5 NATO operations in the Balkans features a center white stripe with silver thread. It is uncertain which specific operations are recognized by the award of this Medal version. The clasp "NON ARTICLE 5" may be awarded with this version.

Medal ribbons awarded for service in ISAF (Afghanistan), Africa, AMIS (Sudan), Iraq (NATO Training Mission), Pakistan, and the Balkans feature 2 white stripes with a silver thread. The clasps, "ISAF," "AFRICA," "AMIS," "NTM-IRAQ," "PAKISTAN," "OUP LIBYA/LIBYE," and "BALKANS" are issued with this ribbon to denote the operation.

Additionally, the clasp "EX YUGOSLAVIA" is awarded with the Medal, but it is uncertain which ribbon it is issued with. It is also known that a Medal is awarded for resolute support service, but it is unknown which ribbon is awarded to recognize this operation. There may be additional clasps awarded with the Medal.

British recipients of the award are not entitled to wear the Eagle Assist, Active Endeavour, Afghanistan, or Iraq Medals.

Subsequent awards for service in a separate NATO operation, originally designated by an Arabic numeral emblem, are now designated with the award of a number superimposed on a bronze square emblem.

There may be additional versions of the Medal which vary by composition and design.
