The Order of the League of Mercy was established to recognize the efforts of the members of the League of Mercy. The League was established by Royal Charter in 1899 to maintain volunteer staff at charity hospitals before the advent of the National Health Service. The League was dismantled shortly after the NHS was established, but was re-formed along with the Order in 1999.

The Cross is conferred in recognition of meritorious service rendered within the League to healthcare services. Appointments are recommended by the Grand President of the League and approved by the sovereign.

Crosses are worn by men on a breast ribbon and by women on a bow.

There may be additional versions that differ in size or manufacturer.
