This is a large neck order with heavy enameling. Obverse seems to have the Roman Crown device, but the reverse has a divided shield with a rampant lion surmounted by 3 stars at the left and a diagonal bar at the right. Have a nagging thought that there might be some Austrian influenece here. Any ideas?
Looks like the arms of the reverse are from the City of Potenza in Southern Italy? I can't see the image of the arms very well on the medal.
Corona Normanna d'Altavilla. It was a private order given in Italy around 1950. I don't know if it is still active.
Have a look at the photo, taken from eBay.
Best regards
