There is no information on the institution of this award. It was awarded in 1943 and 1944 (and possibly also 1942), according to the inscriptions found on specimens.
The award document "Für Hervorragende Leistungen" (for outstanding merits) has no connection to this plaque, unless it was potentially a prerequisite to be awarded the plaque.
The plaque has a 6mm wide border of laurel leaves. The upper half features an Iron Cross inside an octagonal laurel wreath, with a superimposed Luftwaffe eagle with swastika. Underneath is the inscription: for outstanding merits / the commander-in-chief of Luftgau XVII / war year 1943 (or 1944). In between cross and inscription is room for the recipient's rank and name.
The reverse is hollow.
The plaque was awarded in a red case with white silk interior.
