The New Zealand Defence Service Medal was established by Queen Elizabeth II, Queen of New Zealand by Royal Warrant in April, 2011. It was created to recognise military service since September 3, 1945. Ex-servicemen are eligible for the award.

The Royal Warrant allows for 1 or more clasps to be awarded, which denote the type of recognised service. There are four clasps that include "REGULAR," "TERRITORIAL," "C.M.T.," and "NATIONAL SERVICE." The clasp that reads “REGULAR” is awarded for 3 years of service in the regular force, the clasp that reads “TERRITORIAL” is awarded in recognition of 3 years training in the territorial force of New Zealand, the clasp that reads “C.M.T.” is awarded for completion of the recipient’s obligations under the Military Training Act 1949, and the clasp that reads “NATIONAL SERVICE” is awarded for completion of the recipient’s obligations under the National Military Service Act 1961.

The Medal features the recipient’s name engraved on the rim.
