This medal was conceived by Peter the Great to commemorate the battle against the Swedish Army near the town off Lesnaya. The battle took place on September 28, 1708.

The gold medal was conferred upon high-ranking Officers.

The obverse inscription translates to: "Tsar Peter Alekseevich Sovereign of All Russia". The reverse translates to: "to the Deserving of Merit" and "for the Lewenhaupt Battle".
