The Exalted Government Medal (Tamgha-i-Sarkar-i-Alia) was founded by Nawab Sultan Jahan Begum Sahiba. Nawab Sultan Jahan Begum Sahiba ruled as the Begum of Bhopal between 1901 and 1926. During her reign, Nawab Begum brought forth progressive reforms to the fields of Bhopal's education, taxation, military, police, judiciary and jail. The Medal may have been awarded in 2 Classes, but additional research is necessary to confirm.

The Exalted Government Medal (Tamgha-i-Sarkar-i-Alia) is similar in design to the Sultania Medal (Tamgha-i-Sultania).
