This medal was established by Maharaja Bhupendra Singh. It was conferred upon Patiala Imperial Service Troop personnel in recognition of service in the 3rd Afghanistan War and on the North-West Frontier.

The reverse Devanagari and Gurmukhi inscriptions translate to "Afghanistan". It is also dated "1975 [1919]" in Gurmukhi.
