This medal was established by Maharaja Bhupendra Singh. It was conferred upon Patiala Imperial Service Troop personnel in recognition of service during WWI, specifically on the Western Front and in the Middle East.

The reverse Devanagari and Gurmukhi inscriptions translate to "Great War". It is also dated "1970-75 [1914-19 A.D.]".
