This medal was established by Maharaja Bhupendra Singh. It was conferred upon Patiala State Force and Patiala Police Force personnel in recognition of service with the British during conflict in the Punjab.

The reverse Devanagari and Gumukhi inscriptions translate to "Punjab". It is also dated "1975 [1919 A.D.]".
