This decoration is also known as "Pingat Pertabalan" and was instituted by the Sultan 'Abdu'l Halim Mu'azzam Shah on occasion of his installation as Sultan in 1959.

It is composed of white metal with a straight suspension that incorporates the Kedah Crest. The obverse bears the head of the Sultan, facing right, with the words "Sultan Abdul Halim Muazam Shah" above and "Kedah Darul Aman" below, all in Jawi. On the reverse is the Jawi inscription "Pingat Peringatan Pertabalan Duli Yang Maha Mulia Sultan Abdul Halim Muazam Shah" meaning "Medal in Commemoration of the Installation of his Highness Sultan Abdul Halim Muazam Shah February 1959."

The ribbon is yellow with four thin strips of red, green, yellow and red at the edges.
