This award is also known as "Bintang Untok Pekerjaan Lama dan Kelakuan Baik." It was awarded for twenty-one years of continuous service and good conduct to those at the rank of an officer (military division) or executive (civil division). Bars may be awarded for every five years service thereafter and the decoration may be withdrawn under certain circumstances.

The award is a six pointed star suspended by a ring. The obverse central circle bears the State Crest and on the reverse the inscription in Jawi and English "Untok Pekerja Lama Dan Kelakuan Baik" meaning "For Long Service and Good Conduct." The ribbon consists of yellow, green, red, yellow, green and double red, repeated.
