The Long Service and Good Conduct Medal is awarded in recognition of long and meritorious military service. At the time of its creation, it was awarded to Non-Commissioned Officers, military personnel, and nurses, but since 1974, it has been awarded to Non-Commissioned Officers and Warrant Officers in all branches of the Hellenic Armed Forces, as well as the Public Security Corps.

From 1937-1974 the grades were distinguished by composition, but since 1974, the grades have been distinguished by ribbon clasps. From 1937-1974, the recipient's service branch was also distinguished by the ribbon pattern. Army personnel wore their medals on a dark blue ribbon with a red centre stripe, naval personnel wore their medal on a dark blue ribbon with a yellow centre stripe, Air Force personnel wore their medal on a dark blue ribbon with a light blue centre stripe, and nurses wore their medal on a white ribbon with a red centre stripe.

The obverse inscription of the first version translates to “George II King of the Hellenes,” and the obverse inscription of the second version translates to “Greek Republic.” The reverse inscription of both versions translates to “For Successful Service.”
